Submission
You can fill out one questionaire or all of them - whatever makes you more comfortable - or uncomfortable, if you prefer.
Below is a list of D/S activities that you may find
helpful in guiding your servitude. This is a list meant to be
filled out by you, the submissive, for My use.
The list should be used as follows:
- Indicate for each item how you FEEL about that activity
by rating it on a scale of 0 to 5 or as a NO. A response of
NO means
you will NOT do that item under any circumstances. A
zero indicates you have no desire to do that activity
(in fact, may loath it) but you are willing to do it to
please Me. A response of 5 means the activity is a wild turn-on,
with the other numbers indicating some level
in between.
- Indicate for each item your level of experience
by rating it on a scale of 0 to 5. A zero means no experience
at all, 1 means you have tried it once or twice, 5 means
you are very experienced. You may omit those items to which
you answered NO in the previous column.
